Dear Lactnet members,
This is a response/follow-up about the mother and infant who had persistant
yeast. I hope I stated that she had OALR and abundant milk supply. This
would explain why the baby was gassy and pulling off the breast, fussing,
and not being fussy or gassy when given BM in a bottle. Perhaps I did not
make that clear in my original post. I think my focus changed from
reducing the mother's supply (nursing every 3 hours per breast, pumping
slightly, then advancing to longer at the breast, etc.), to the yeast
problem. In the process, I forgot somewhat about the OALR, because I
thought it was remedied.

The mother was sent to another ped in a large teaching facility, which she
considered a waste of time. Since then, she has weaned the baby, and has
not seen a reduction in the yeast in either herself or the baby. She has
to have her wisdom teeth out soon, and will have to be on ABX for that, so
that weighed in her decision. She plans to go on a strict anti-yeast diet
during that time and continue Diflucan. In the meantime, the baby is not
tolerating the ABM well. I will let you know what happens. I did convey
to her that it would be possible to relactate, and she plans to continue to
come to the bf support group, so we shall see.

Thank you to everyone who answered my post.
Warmly,
Sharon Gollman, RN, BSN, IBCLC
