After the Wall Street Journal article surfaced (July 22, 1994 in case anyone
cares), I started looking into the 5%.  One reference was Neville & Neifert's
original book, published 1983 in which Neifert says on page 303, I believe,
that up to 5% of mothers are unable to produce enough milk for their infants.
 The quote is not referenced.  I called LLL to talk to Betty Crase -- a wise
woman who knows A LOT! -- and she was looking for the very same "where did
this originally come from" sort of thing.  She found it in a 1971 book called
Breastfeeding, The Tender Gift by Dana Raphael.  It referenced an article
from 1958 which Betty was able to find -- that article referenced an article
from the 30's which has since been lost in the antiquity of time.

So there you have it.  The genesis of that 5%.
