What about vasospasm?  Have you observed the moms nipples 20 minutes
after pumping to look for blanching?  Has she tried a nipple shield?  In
this case, where she is barely able to put the baby to breast, maybe
they will help provide the comfort she needs.
I doubt psychological issues are a major source of breast or nipple pain
(though they certainly play into tolerance of pain), keep looking.
