Transition Nursery

Hello
Well, just when I thought things were getting better, I learn we are going
to leap way back in mother baby care.

The neonatologist and some of the peds want a 'transitional nursery'.  They
believe newborn problems are being missed immediately after birth.  And they
propose a 4 hr mandatory transitional nursery stay for all newborns
immediately post delivery.

I know there are instances when the physicians have been rightly concerned
about poor infant assessment by staff.  I recognize that the present method
of 'herding up' all the babies each morning and parking them in the nursery
until the peds are finished with morning assessments is a nightmare when it
comes to one nursery nurse being able to handle newborn needs, peds
questions, and physician needs.

There are a host of problems that need to be addressed and changed to
reduce problems.  But...The mother baby separation issue is such a backward
step...especially for breastfeeding. I believe we need to look at the
present problems and work on those before we take a drastic measure like a
mandatory transitional nursery stay.

I am part of the management team and we are trying to put together an
alternative solution to take to these physicians.  We are trying to get our
ducks in a row...fast.  And I'm looking for your input.

FYI...Delivery rate here is 2000/year.

Questions for you:

Does your hospital have a mandatory transitional nursery?  If so, how does
it work?

If you have a transitional nursery, what are the benefits/drawbacks?

If you have a transitional nursery, what are the pt satisfaction responses?

How does a transitional nursery impact breastfeeding initiation and success?

Do you work were there is an assessment team that goes to the rooms post
delivery and watches over the babies for a few hours after birth?  How does
that work?

If you are able to respond...please add any thoughts/hints on presenting a
case to counter this.
