I think this is a great question and I will be interested in hearing what
other people do.

I think I have made different suggestions in different situations, depending
on how the baby seems to be doing and what seems to be happening with
mother's milk supply. Most often, I suggest feeding the baby at the breast,
both sides, doing breast compression too, and then if the baby still seems
hungry, add the feeding tube and let the baby take as much as he or she
wants (still at the breast). As the mother's supply increases, the baby will
take less and less from the feeding tube and ultimately (we hope) will be
completely satisfied at the breast. I like this approach because I like the
idea of the baby deciding how much to drink at each feeding.
