Pat and others: I have a medicaid provider #. Actually very easy to obtain.
Just called and requested forms, then filled out as I am a DME supplier:
breastpump rental station.
Unfortunately filling out medicaid paperwork by hand is tedious and
difficult. Of course, now in Indiana they are not paying for rental pumps,
only a hand pump so I am not billing much this year (thankfully). Without
the computer programs to electronically submit, refusalls are common.
Actually, get the providership number was surprisingly easy!
