A prolactin level of 3 is devastating for milk production; 8-15 or so is
normal for menstruating women who aren't even lactating.  It is important
for the mother to see an endocrinologist ASAP, and to continue pumping 8x a
day even if nothing is coming out for the time being. They can try
domperidone and see if that helps. If not, google Corinne Welt MD in Boston,
who is doing prolactin replacement therapy testing. There's an excellent
chance that this would get milk production going, though unfortunately only
for the duration of the test. 

I just had a mom who successfully breastfed her first and had #2 about 6
weeks ago. Milk hardly came in, herbs didn't do much. Her prolactin came
back at 2.2, and of course we are asking what the heck happened between the
first and second baby!  An MRI revealed a "nodule" on her pituitary that the
endocrinologist deemed insignificant. He only reviewed the pics by phone and
didn't want to see her.  Domperidone stimulated milk production and brought
prolactin up to 67, but then the mom gained something like 10-20lbs within
the couple of weeks she took it (that is NOT normal!), and so they took her
off and are going to do a thorough investigation.  Unfortunately, her FP doc
recommended she wean so that they could figure things out.  :-(   I'm hoping
to hear the final conclusion.
