A mother with bi-polar disorder, nursing 13-month-old twins, has asked
for information on Trileptal (oxcarbazepine) -- Margaret Wills, LLLL, IBCLC


I don't have any direct info on this drug.  I did have a bipolar client
nursing a 2 year old who switched meds a couple of times to newer drugs.
Sometimes there wasn't much info.  I would always give her any info I could
find; usually it was pretty benign.  Her docs were absolutely against her
bf.  Her point was that at that age (as with the twins), breastmilk is not
the main source of food anymore, and any drug excreted in the milk was
probably negligible.  Is the drug approved for pediatric use?

Her other point was that when her mood was not stable, breastfeeding was
one thing she could do that really made her feel good.  She was worried
that weaning before she was ready to could send her into a mood change -
most likely severe depression.

With each drug change, she signed a paper holding the psychiatrist harmless
if her child developed symptoms.

I am NOT recommending your client take a drug and breastfeed if it isn't
safe.  This was her experience; she wanted to be able to make an informed
decision.  She did refuse a few drugs that she didn't take because of
potential risk.  Since her psychiatrist and associated pharmacist were
against her bf, I was her source of info on the meds.  I did not recommend
for or against, I only gave her the info, spoke with her doctor, and gave
the doc copies of the info I'd found in Hale, etc.
