I tried Reglan with a couple of my adopted kids, and had too much trouble
with depression to stay on it. I felt like it helped some with milk supply,
but definitely not enough to merit dealing with the increase in depression.
For quite a few years, I would run into adoptive moms who had used it but
almost no one felt they could deal with the depression and/or anxiety it
caused them. A few of those had very serious problems that took months to
get over, after they stopped taking it. I don't recommend it to anyone.

What I would do, for the prospective adoptive mom, is make sure she knows
that she can get a Lact-Aid and just start nursing the baby with it, whether
she has any milk to start with, or not, and then tell her what the options
are for trying to start milk production in advance. If I was going to be
adopting another baby, given what is available at this point in time, I
would probably go ahead and start taking domperidone and pumping a couple
times a day, fairly soon, and then play it by ear, as far as increasing the
pumping and such. It is always good to consider the fact that many planned
adoptive placements do not end up going through. That is one reason that I
think the all out efforts, like the Newman protocol, are best considered for
surrogacy situations, where the placement of a baby is much more certain.
The adoptive mom who has been pumping a few ounces a day and freezing it can
still offer her baby a significant amount of breast milk from the start,
while maintaining a situation where it is easier to stop pumping, if she
ends up with no baby and feels the need to avoid the subject for a while.

Of course, another option is pumping alone, although it is important to know
that moms who pump in advance, without taking domperidone, might not see
much milk until after they are able to replace the pump with a baby at the
breast. Some moms have given up before they even had a chance to nurse a
baby because they thought that what they got from a pump was an indication
of what they would produce for a baby at the breast. There have been moms
who have pumped for weeks without seeing any more than droplets, but have
been producing ounces quite soon after they started spending a great deal of
time suckling a baby with a supplementer.

I wish the prospective mom the best! Breastfeeding my adopted kids has been
the most wonderful thing I have ever done!
