I have worked with several very large breasted and alrge women. The best
results seem to be when the mother is sitting up very straight, with several
pillows ( firm) beside her and then sitting the baby on those, facing mom.
Baby is sitting almost upright. Then after latchon, put a pillow behind baby
to keep him from sliding down. Mom has to hold her breast and nipple in
opposite hand, raising the nipple up to the baby and then latching on.
