Wondering what other NICU LCs use for documenting their interaction with the mom and baby, and/or assessment of feeding at the breast.  At our hospital, we don't have a designated LC for the NICU--all of us work with the mothers on pumping, and the mom or the baby's nurse ask us to assist with feedings as needed.  Do any of you use a form that goes on the baby's chart?  I've seen journal article references that I'll be checking on in the medical library (one by Baker and Rasmussen I think has something about documentation), but thought surely someone on here has a documentation tool for NICU.
