Do any hospital LCs have a breastfeeding "care plan" that you use to
communicate with the nurses when a mom is struggling with breastfeeding?  I
would like to communicate better with the nurses when I am not there, and
would like them to follow up better with the plans that the mom and I put in
place. (I do chart everything in the nurses' notes, but the nurses never
seem to look at it---so I want something separate that stands out.)

I have a care plan that I give to struggling mothers (that I also plan to
revamp) but for some reason---probably the stressful situation when the baby
won't latch---many mothers forget I even gave them a written plan let alone
remember to share it with the nurses.  So, in a nutshell, I am going to
develop a communication page for the nurses off shift, so that we can all
support the struggling breastfeeding mother with the same plan---and I am
asking if there is anything already out there that anyone would be willing
to share.
