Have you considered the possibility of thrush? These symptoms sound like a
classic case to me, where the pain is deep in the breast and goes on for
long periods after the feed. This can happen when thrush enters the milk
ducts. The Breastfeeding Network in the UK suggests the following treatment
if this condition is confirmed:
Nystatin tablets 500,000 units every 6 hours for 14 days
or
Fluconazole capsules, 150mg to start, then 50mg twice a day for 10 days.

Perhaps the mother or the baby has had antibiotics recently?
