We have a full sized upright freezer for milk in our Level III NICU. We use plastic baskets tape-labeled with the patient's surname on each one. It's that same tape used on the outside of 3-ring binder medical files so they are easily changed as baby is discharged (or the milk runs out). Mom brings in her frozen milk and puts the new behind the old.

All her bottles are labeled with the infant's ID sticker with date/time are hand written on the edge. And they are STILL stored in those awful Biohazard bags on edict of Infection Control. We also have two smaller refrigerators - table-top variety. One is for prepared formula and one is for fresh milk or defrosted milk to be used that day.

Nurses or the HUC provide mom with new bottles and a sheet of new labels as she needs them. If mom brings in bottles with hand-written ID or masking tape, the HUC or the RN will make sure that hospital ID labels are placed on each bottle before putting the bottles into the freezer. That is done in mom's presence so she witnesses the labeling process.

We have a standard practice of double identification of every single milk feed as a prevention measure - to avoid feeding wrong milk to wroing baby. There is a place on the Nurses' Notes (where all feeds are recorded) where the initials of those 2 RNs are noted. Any time there is a question related to any milk feed, those two specific RNs can be contacted.

These questions are for all of the hospital LC's out there. In the past week we have had two incidents where babies recieved or potentially recieved the wrong ebm. My director sends you these two questions.

1)	Is there a breast milk refrigerator that everyone likes (for storing multiple patient’s EBM)
2)	Is there a product out there that would allow mom to print out her own labels to ensure that her cups are labeled with her baby’s name? Currently it is the nurse that gives moms a bunch of labels to use at home.

Thank you so much. Ellen
