About 2 months ago I posted about two mothers:

#1 had sore nipples that were not healing despite our best efforts on all
fronts.  The only thing not done may have been the most pertinent thing to
do: her nipples were never cultured.

Baby didn't deform her nipple in any way, but the nipple tips stayed badly
eroded and simply didn't heal.  Neither topical nor systemic treatment for
thrush or Staph aureus helped.

What finally worked was just plain time.  The mom hung together (barely) for
about 7 weeks, by which point the damage was nearly healed and the pain was
entirely gone.  She had major problems with her first baby as well, which
ultimately resolved around 6-8 weeks.  I had assured her that this time
couldn't *possibly* be as bad...

#2 was a diabetic mom who had pneumonia when she delivered her 2nd child.
First was breastfed into toddlerhood, though with not-quite-adequate supply
in the early months.  But this time she couldn't even express drops.

She's now feeling much better, is breastfeeding with a supplementer, has
recently started Domperidone.  Her supply has risen, in the past 6 weeks or
so, from hand expressing nothing to hand expressing drops to being able to
hand express one or two tiny spurts.  The pump still produces nothing.  Her
insulin needs remain about half of her usual and she still doesn't have the
"dawn phenomenon", or early-morning rise in blood glucose, that is normal
for all of us and was always normal for her.  Something's still out of whack
somewhere.  But life proceeds...
