We do have proof that folic acid is helpful in decreasing the number of spina bifida birth defects, and maybe cleft lip/palate, and 1mg is more than double the RDI (recommended daily intake).   I have not seen anything to suggest that 4mg is necessary.  AND, just so you all know, water soluble vitamins when given in excess may be excreted in urine, but excessive doses overload the system, and these vitamins can accumulate and act as toxins.  B6 causes neurological damage and hair loss, for example.
   So if I were that mom, I would eat a very healthy diet of organic foods, and include food sources of folic acid every day, and stick to 1mg per day, since the recommended amount is  0.4 mg.
