Hi wise ones, I need help with a very dedicated mom who has OALR,
oversupply and persistent plugged ducts. We are dealing with multiple
issues, but mom really wants to get supply down. She is considering
involuting one side just to regulate supply. I suggested she continue to
downsize supply instead. She decided to use sage tincture, taking 5-10
drops TID. Here's my question: How long before she should see improvement
with supply, or should up the dosage? It's been about 48 hours with no
improvement. She now feeds one side per feeding(q 2-3 hr), but breast rock
hard by the next feed. She tried for 24 hrs to go 6 hours per side but
doesn't want to repeat that. Any other ideas or suggestions about supply,
or something I'm missing?
Hx: Baby is 5 weeks, home birth, primip mom came to see me at 3 weeks, dx
with mastitis x 2 (tx w/Keflex, then diclox.). I observed raw nipples,
OALR, oversupply, and yeast. Mom was feeding on one side and pumping up to
10 oz out of other. I gave her shield which helped slow flow. She lets down
every few minutes and milk pours out of the shield and baby's mouth. Fast
forward 2 weeks: Baby has finally stopped clamping, mom still using shield,
nipples healed, no sx of yeast, baby can soften one breast per feed. For 2
weeks now she has taken following suppls: lecithin 1200mg QID, Cal/Mag BID,
Bromelain TID, Grapefruit Seed Extract, Vit C, Golden Seal, Iron, PNV. Her
husband is using accupunture to help w/yeast. She uses cabbage compresses
and grated potato poultices daily. She has not pumped in 2 wks.
Any suggestions are much appreciated.
