When we had this problem locally - inappropriate doling out of
shields with no information about drawbacks and no follow up - I
wrote to the director of midwifery at the hospital concerned and told
her about it.

I didn't name names, I just told her it was going on, and backed up
my letter with references.

She wrote back, thanking me, and since then the problem has not been
anything like as great...in fact, in about a year I don't think I
have had one mother who gave birth in that hospital calling me to ask
how she can get rid of it, or telling me her milk is dwindling after
using it.  I think it helped that the director knows me, and we have
good relationships with her.

I don't know about the hierachical 'chain of command' in US maternity
units, but  maybe a letter to to the top will get results. You don't
need to say you know it is a particular professional - in any case,
she may not be the only one.
