This week I talked to a pregnant mom with an unusual problem,, who is coming
to see me later this month. I was hoping some of you might have good input.
At age 12, mom had surgery to remove multiple accessory nipples bilaterally.
She was also at a later date told by a plastic surgeon that she "did not have
enough breast tissue" to make milk. With her first pregnancy,4 years ago, she
experienced breast enlargement bilaterally, but tells me that "my milk only
came in on one side." She says that the other breast never leaked colostrum
or felt fuller. She ended up feeding formula after a week, a decision that
she says she came to largely because of dad's anxiety that the baby was not
getting enough milk. (Sorry I don't know more details about her supply.)
She is now 35 weeks into her second pregnancy, and is leaking colostrum,
enough so that she needs breast pads, from both breasts. On the breast that
didn't seem to produce last time, however, the milk is leaking from several
sites where accessory nipples were removed, and not from the nipple itself.
She says that at least two of these sites are within an inch of the actual
nipple.
I am curious if any of you have ever helped a mom with a similar problem.Why
is she leaking from the accessorys and not the actual nipple? Mom and Dad are
coming to see me before delivery, and I'm hoping to give him some confidence
in judging supply adequacy. I also am interested in evaluating mom's anatomy-
if baby has a wide latch that includes the accessory nipple sites, do you
think nursing on that side might be possible?
