When my children received vaccinations I usually held them securely in a
"cuddle-hold" in the sling.  I would free the necessary thigh first.  The
baby was close to the breast for comfort and with some gentle soothing was
almost always calmer than when she was sprawled out on the table.

I tend to agree that an unsuspecting nursling may form a negative impression
when jabbed in the thigh while doing something that is supposed to be
comforting and secure.   Might this cause a nursing strike?
