Hi,
I am currently trying to avoid reinventing the wheel and am looking at
sources for consult forms, care plans etc.. I am using childbirth graphics
for care plans but need more than they offer and have made up my own intake
and consent forms but they need tweaking. I am considering getting the
forms bundle from Lactation Education Resources in Maryland ( the old
Georgetown program)
Is anyone using them and like to offer an opinion? Anyone have other forms
they like? I run ( ok I am) the breastfeeding resource center at the local
hospital and I am desperately trying to increase the level of
professionalism and would rather have preprinted handouts than the
individual hand written ones I do now and then have to go and make copies of
for the DR report and my records. Thanks for the input.
Question number 2 is off topic- Could the herbalists on lactnet let me know
where/how they got their certification? I have a friend who is interested
