Yes I have done some work on this as Heather said - and sorry for not
responding sooner, life is *very* busy and stressful these days.  I
recommend the April 99 Paediatrics paper already mentioned.  Should be
easily used by anyone in the US, if you assume that the rates of
hospitalisation/office visits are similar to those quoted.  Since they quote
in costs per 1000 births, it is straightforward to scale up to match your
local number of births.
