I am a new IBCLC but have been a baby nurse for many years. I recently
started working for a home health agency doing mom/baby visits on short
hospital stays.  When the mom was breastfeeding, I would do an assessment and
make suggestions based on what was happening.  Now that I am an IBCLC, my
agency wants me to do follow up visits on breastfeeding moms that are having
problems.  The pay is next to nothing to do a follow up visit because it
should be a short visit. The inital assesment should have already been done.
You all know that to do a breastfeeding assessment and observe a feeding will
take at least a couple of hours.  They are not willing to pay any more than a
follow up visit rate for these visits.  I would like to know how you think I
should approach this. I would like to do the visits, but will not do them for
nothing. My time and expertice is worth something plus I don't want to set a
precident.  The other problem I have is determining what I should ask to be
paid. I am not sure what reinburcement is like from insurance companies so I
don't know what is fair to ask.
