Bracha
This is one tough cookie! The 4.5 mo old could have bottles and pacifiers
(dummies) if any, removed. The child could be cup fed or even a sippee cup
used. This may encourage the baby to fulfill the sucking needs at the
breast. I would do alot of cudddling, cosleeping, skin to skin, but no
forcing of the issue. Keep up the milk supply, of course, that is essential.
I would certainly continue to rule out medical problems, as I see the family
has already done some of this.
