There is another option that can be used when there are two adults in the
car.  If mom can move to the back seat then she can simply lean over the car
seat (a rear facing one) with only her lap belt on and nurse the baby with
everyone buckled.  Mom may be slightly less safe than if she was wearing her
shoulder belt, but if you simply can't stop (or don't want to take the time)
this can work.
