Dear Joan,

I suspect that there is no pat advice on schedules, etc. that will
necessarily help this mom.  Don't mean to be flip, but it seems
kind of common sense to me.  Don't see where pumping has any
place here at all, unless the babies have a problem with feeding,
since mom will be getting more than enough stimulation from two
babies at the same breast.  She feeds them when they are hungry -
 if both want to eat at the same time, then will have to pacify one
while the other nurses.  Don't really think that the foremilk/hindmilk
thing is such a big deal, but may want to alternate who goes to
breast first.  But I don't think that the breast will be unused long
enough for this to be an issue.  She will be nursing very frequently,
so needs to be realistic about how much time she will have to be
doing much of anything else.  This will probably be like a two-
breasted mom feeding triplets or any mother of twins who doesn't
feed the babies at the same time - archives contain a lot of
resource ideas for moms of multiples which will help this mom no
matter how many breasts she has (?-you know what I mean).  Her
breast will most likely ramp up quite quickly to produce whatever
amount of milk is necessary to sustain both babies.

Again, no need to get too technical here.  There will be plenty of
colostrum to go around.  Give her all the standard resources for
moms of multiples, hook her up with a mentor mom who has
nursed multiples and don't let her focus so much on the one-breast
issue - I really don't think it will be that big of a factor except that
she won't be able to feed them simultaneously - but many twin
moms don't do this anyways, even with two breasts.

Obviously, I don't have a lot of personal experience with this.  But
since you were begging for a reply, thought I'd offer up something.
Really, I think she will just need the same kind of support that any
new breastfeeding mother craves.  Good luck.
