Last night, I cared for a woman who is 2 days postpartum with fibrocystic
breast disease.  Her breasts are so full of cysts that it is darn near
impossible for her to get the baby latched onto the breast because her
breasts, areola and nipples are so rigid.   She presently is pumping and
finger feeding.  Her breasts do not soften at all with pumping.  When I saw
the baby this am, she was yellow to the buttocks and am sure a bilirubin
will be done today.  Mom is very motivated to breastfeed.

I'm stumped.  What can we do to facilitate nursing directly from the
breast?  Am I being impatient?  We plan to refer her onto an IBCLC in our
community as part of her d/c plan as we have a lot of CBEs at our hospital
but no IBCLC.  HELP!!!
