Hi everyone, there sure has been a lot to learn in the last several months
with all of the different discussions we all have been having. Anyway I need
your help. I have a mom with her second baby that came to see me about why
her baby always vomits once a day. Mom can narrow it down to the 6 pm or 9pm
feeding and always a feeding that is a "good one". Baby doesn't just urp up a
little with a burp but full fledge vomit, out the nose and even recently while
still latched to the breast although mom says it is usually when she changes
the position to burp her. Mom reports that baby nurses about 5 min per side
but her other baby did just the same. Good latch on visual assessment. Mom
is very conscious about her diet and drinks a lot of water and rarely apple
juice and only has rice milk for breakfast with her cereal. Baby is gaining
weight and is 7 weeks old currently. the vomiting has been going on for a
while but mom thought that when the baby was new it maybe wasn't so much of
the feeding and that things would get better. At her two week check she had
maintained her birth weight and at 4 weeks had gained a pound. Baby does have
what a PA in my office diagnosed as inclusion cysts on her upper and lower
gums. They look like white pinhead size "pimples" that do not come off with
touching them. No other signs of thrush. Mom reports eating a fairly bland
diet but seems like a lot of carbohydrates? Any thoughts? Mom is just
worried about why she always vomits and at almost a predictable time of day.
Any help would be greatly appreciated.
Thanks,
Toni Myers RN, IBCLC
