Kym,
This is what we call a "train wreck!"
I had a somewhat similar client who experienced postpartum hemorrhage,
shock, transfusions, and then to top it off was put on hormonal birth
control right away. She used herbs and metoclopromide (reglan) and an
SNS. Never did get a full supply.
In your client's case I would recommend:
1. feed the baby via nursing supplementer at breast - use ebm as
available and formula; add breast compression; d/c the cereal, as 8 wks
is too young
2. pumping the breast and hand expression on a regular schedule and use
whatever is obtained post feed in the next supplementer
3. other herbs if fenugreek not liked - blessed thistle, I forget the
others - check archives
4. share info with her and physician about metoclopromide - this would
be a decision made after consultation with the doc. weighing risk vs
benefit in light of the depression
5. decrease smoking as much as possible if not able to quit
6. improve diet and calorie intake of mom - WIC can help greatly here -
probably the whole family could improve here
7. when the depo is gone, use non-hormonal birth control is poss.
8. household help if possible - or could she go to LLL mtgs with baby
and toddler in tow?
This lady probably will not achieve full milk supply to be able to
exclusively bf the baby, but maybe when the baby is older can d/c the
supplementer. Some br. milk and baby at breast is better than none. Good
