Agreed - neither of these terms is either accurate or helpful. Chloe
Fisher's work says something like 'finish the first breast first' and
explains that this means letting the baby decide when he/she has had enough
of that breast.

There is no reason why a baby should be 'made' to stay on the breast longer
than h/she appears to want (I'm talking healthy, thriving babies here, of
course),  is there?

I have found that while it's important to know about hindmilk and foremilk,
mothers worry about it, as do some HPs, unnecessarily.  For example, a
mother will ask 'he only stays on for X minutes - surely he can't be
getting the hindmilk?'

Mostly, healthy newborns can be trusted to take what they need from the
first breast in terms of time, foremilk, hindmilk...we don't need to be
asking mothers to do odd, unphysiological things like 'emptying'...do we???

I reassure mothers that letting the baby decide how much he wants of the
first side, and then offering him the second and not worrying whether or
not he takes it, is fine.
