I learned to trim out pie shaped wedges on each side to allow the baby's mouth
to get on nipple.  The shield is gradually trimmed away, disappears and baby
is on the breast!  I have not tried this myself but understand it is used by
some local LC's at a hospital in our area.  Don't know of any studies on any
cutting procedures or safety related to baby's mouth being cut.
