 We use crib cards at the hospital where I work that are supplied by a formula
 company.  We write the name in bold magic marker across the top and also in
 bold marker the form of feeding in lower right corner.>>

We also use formula co. supplied crib cards at the head of the crib where the
info is filled out in regular ink.  Hard to read unless you get up close &
personal.  However, on the side of the bassinet is either a salmon colored 3x5
card or a white one.  It has the baby's first and last name & mom's room
number written on it on both sides.  The salmon colored card means the baby is
breastfeeding.  The white card is for bottle feeding.  Fortunately there are
lots more salmon cards than white cards!  Of course, occasionally we get the
mother that is both "breast & bottle"  In that case, they cut the cards in
half so they get half a salmon and half a white card.  Duh.  But basically, I
love the system.  Quick info at a glance.
