Hi Lori. Here in New York City every baby gets a yellow card that folds up
to 8.5x4 inches, titled Lifetime Health Record. It has sections labeled
"Lifetime Immunizations, Dates and Signatures," "History" -- including among
other items a little box labeled "Breastfed ___ months" -- "Special Care
Needs," "Screenings," and "Health Care Visits" with places to mark weight,
height, diagnosis etc.
These cards are produced by the New York City Department of Health. My two
daughters, born 3 years apart in 2 different hospitals, have virtually
identical cards. I bet you could get copies of them from there (of if you
want photocopies of mine, email me privately). Maybe you could persuade your
local health department to do them up too? We got formula company ones too
but since the hospital and pediatrician had already entered the birth stuff in
the yellow card we just threw the formula ones away, as I suspect nearly
everybody in NYC does :).
