I am not opposed to the use of nipple shields in general but I don't
think that using them in the initial days after birth is helpful. I
reserve them for when nothing else is working and then I inform the mom
that this is a short term fix, not something she can use for more than a
few days.
This infant is probably not gaining because the moms supply is
significantly diminished due to the early and continued use of the
shield.
I would have mom start pumping with a double electric breast pump and
begin fenugreek asap and start weaning the baby from the nipple shield,
try the SNS without the shield.
