Gee, Kathy, I was told that PhD stood for Phony Doctor. ;-) I have found
at the medical center here when an MD addresses me by my first name and I
then refer to them by their first name, they really don't seem to mind.
But then again, with a name like mine perhaps they would prefer to be
addressed by their first name just so they don't have to pronounce my last
name.
