Kathleen,

I really don't think you're missing a thing.  The baby probably got mom off to
a bad start in the hospital, sleepy at the breast by day 3 if not in the
beginning.  She was using an inappropriate pump until a day and a half ago.
And mom's under stress from work.  (Hasn't she heard of maternity leave?
She's only 7 days postpartum!)  Of course, she's not getting enough milk with
the pump yet.  She needs to rest, eat well, pump frequently, and feed the
baby.  And what is the bili?  The new recommendations are not to treat until
the total is around 20, but if the baby is very sleepy, perhaps a bili blanket
is needed.  Is the baby at least sleeping with just a diaper near a bright
window (not in direct sunlight)?
She successfully breastfed 2 other children.  What's going on this time?
Unplanned baby interfering with her career?  I'm not there, of course.  But
she has verbalized throwing in the towel after less than a 2 day effort.
