1st-if I remember correctly twins pg can cause elevated AFP test.  If there
is any lingering concern over babies' downs status I would hope the HCP
would do a chromosome study.  It's better to know for early intervention in
the developmental dept.  Mom can ask for test on each baby.
2nd -are they fraternal twins?  If so the size diff would just be normal
for them as individual sisters.  Also what do the parents look like? Are
they petite?  What have other children looked like as babies? Petite?
3rd-4 oz week is an OK weight gain, and that's just exactly what some BF
babies do.
Of course in  this case it might be 6 sibs that have an effect on actual
nursing (not truly optimum # of times per day, babies put off briefly at
each feed will  eventually accumulate enough time to equal at least one
full missed feed per day and this may be the underlying problem.
Especially if smaller baby is undemanding).
4th- if they are basically healthy and developmentally on target - forget
the weight.  They will catch-up when they need to.
