Dear Margery, the HMOs are probably using a "formulary".  It probably says
you only need diflucan for mom times 1 dose.  One of our formularies
doesn't even know about liquid diflucan for infants.    Anyway you, Dr or
pt, should be able to contact the HMO's Rx board and ask for a
non-formulary drug use.  It's a pain in the ----, takes about 45 min on the
phone, but I haven't had a refusal yet when I explain the situation.  Good
