Milk secreted onto the nipple lubricates it, preventing friction
injury. When there is no milk secretion (pumping to induce lactation),
the nipple can be injured by friction against the pump. Without a
pregnancy, mom lacks the hormonal mileu that causes the breast skin to
relax, and be more stretchy, so the pull of the pump can cause pain. I
usually recommend lubricating the nipple with water or lansinoh, and
pumping for no more than 5 minutes at a time until a few drops of milk
begin to be secreted, at which time pumping duration can be safely
increased.
