Dear Fellow Lacnetters,
I have a mom/baby I am having great difficulty with and need your input. Mom
delivered 1/19/98 with terrible history. 2nd child in one hospital with
ruptured appendix and in ICU. Mom getting toxemia, went to her doc 2 hrs away
for check up and ended up with c/s that day. Mom d/c's 48 hrs post c/s to go
back to her daughter. Anyway called me 1 week post for a sns due to low milk,
mom felt from stress I agreed. Did not hear from them for about 2 weeks, then
they called for a consult. Baby at 3 weeks was 1 lb under birth weight, Nurse
for hours. Every feeding last at least 1 hour using a SNS. AC/PC weight gain
after this feeding was 45cc. Baby has classic breastfeeding reflux signs.
Nurses a few minutes then takes about 3-4 strong sucks, then flutter suck and
falls asleep at the breast. MD placed baby on reflux meds. Mom can only pump
about 1/2 oz at a time. We did all the things to up supply and all the stuff
for reflux. In the meantime, Mom has had a pp infection and ended up in
surgery last week for c/s incision repair. It was gaping.
This baby also has a bubble on his palate. right behind the gum line. Very
little suction and does a lot of chewing motion. Also has trouble with any
bottle we have tried. Yesterday, mom ask me " When do I quit bleeding? I
have ask several times how her bleeding was and she said tapering off nicely.
Well, she is soaking 2-3 pads a day most days and none others but did not
think to tell anyone. So now she is going back to her md today probably for a
d&c.
This mom has so much going wrong, it is hard to tell exactly where the problem
is. I will take all suggestions. First what can we do to overcome this
palate problem? He has a good latch but little suction. Mom is exhausted
from all of this but very determined. I think if we could fix the suction
problem it would help greatly.
