In order to avoid breastmilk being given to the wrong baby, our facility is
looking into designing a policy which would require 2 nurses to check that
the baby's name and the name on the container matched.  I am looking for
others who have this policy in effect.  In particular, I would appreciate a
copy of the policy and record/documentation sheet, if one is being used.
