I can't address all of these points right now but there is a lot here
that sounds like mismanagement and not true insufficient milk syndrome.
One of the things that I noticed was the poor response to the pump.  Many
larger breasted women need a different flange.  There is a glass insert
available that helps some women.  IF the lactiferous sinuses are a little
bit further back than average neither the babies mouth nor the pump will
be adequately draining them.  The behavior of the baby at the breast
after the first day or so sounds like the typical behavior of an infant
conditioned to the fast flow of the artificial nipple.  I would suggest
that the mom try to find a good IBCLC and also a very experienced LLL
leader.  She needs lots of support and should try to line it up now.
