Dear Lactnet friends.
I haven't been active on this list for some time due to serious health
problems (cancer) but need some info ASAP. A CNM colleague of mine called
to ask about the use of diflucan for ductal yeast. She has a patient who is
"in a real mess" after her baby was treated for Beta Strep. The baby
developed yeast and now the mom has raw burning nipples and deep breast
pain. She is three weeks postpartum. The local LC told the mom about using
diflucan but cannot prescribe as she is only an RN. The CNM has info that I
had passed onto her about a treatment regimen I had seen posted here for
ductal yeast (I believe but don't have it right here where I can look at it
that the regimen recommended was 200 mg loading dose of diflucan on day one
followed by 100 mg per day x 14 days).
Does someone have a protocol that they can share for this? Does someone have
documentation regarding the use of diflucan for tx'ing ductal yeast? I told
the CNM I would ask about this. She brought up a concern about the LC's
suggesting medication but not having prescriptive authority and not having
enough personal info or experience to prescribe the suggested regimen.
She has also asked me for protocols on galactogues. Does someone have a
practice guideline they have written for the use of Reglan?
