That sounds like a great thing for both the  LC and the peer counselor.
We are lucky enough to have 3 PC's in our town , the health dept. is
adjacent to the  hospital. The nutritionist at the HD is very
knowledgeable about bf and the 4 of us have a super rapport. The
nutritionist forwards a mini pt profile to the peer counselor and she
make phone contact with the moms that plan to bf before and after del.
The HD flags the charts of the WIC moms that plan to bf because the HD
prenatal charts stay in L and D , except for Wednesdays, which is clinic
day . I make follow-up calls to all bf moms from the hosp , and if
someone needs extra support and encouragement , I call her PC and they
will go out and see the mom. WE also have a HT (Home Tomorrow) nurse
based from our OB unit, any medicaid pts that are d/c prior to 48 hours
, she makes a home visit. We really encourage them to stay as long as we
can , but some just wanna get out of there. This system works well for
us. The PC are great , know their limitations and know when to call for
help.
