I saw that article from Pediatr Infect Dis J, 1998:17:53-8 and I am concerned.
My past advice to mothers and RNs was that breastfeeding with CMV was OK, even
in premies, unless Mom had early, documented primary CMV, such that the infant
would be getting exposed to the virus without much in the way of maternal
antibodies.
This study concerns me in that 14% of the exposed VLBW infants becaume
significantly symptomatic. I still think that for full term infants, and for
larger, more mature premies, the benefits of BM GREATLY outweigh the risks of
CMV. I am pulling some of the supporting references to see whether we need to
freeze our BM for 3 days; test all Moms for CMV; or start our own BM Bank to
pastuerize BM for premies < 1500 gms.
BM is literally lifesaving for tiny premies, and FRESH BM the best. I will be
convening a group to review everything with me. If we reach some conclusions
I'll post it. Would appreciate anyone elses input as well!
