Debbie,
How about setting up your questionaire based on the 10 steps for successful
breastfeeding?  One of the questions could read....'were you informed of
the benefits of breastfeeding? Prior to NICU/During NICU/Not at all...", or
 "Were you informed of how to maintain lactation in babies absence?"

Unicef Baby Friendly also has a self questionaire that asks the questions
about your department/hospital that maybe can help you set direction for
what to improve also.

While questioning the mother's feelings/perceptions of their care is
helpful information, unless you have a specific plan/goal of where you want
to be your questions may not get you the precise info you need.  They may
only be 'fluff' stuff, warm fuzzy kinda stuff.
