I usually can get these babies back to breast easily with a nipple shield (OK
everyone don't scream at me all at once).
Have mom use the shield at every feeding for 24 hours.  She should not let
baby become overly hungry or upset.  Avoid pre feeding stimulation.  Tell not
to fight with the baby--ever.
I tell moms to keep strict written feeding and output records.  Of particular
concern is a decrease in the number of stools.  Serial weight checks while she
is using the shield are a requirement of our nipple shield protocol.
After 24 hours on the shield begin weaning the baby from the shield by--
1.  Starting without the shield
2.  Slipping it out mid feed
Always trying but never insisting.  Once she begins to "fight with the baby"
the battle is lost.  I tell the moms take it one feeding at a time.
This works for 99.9 percent of the babies with this problem.  Some in as
little as one day
