Have had great success with two babies in the last few days, who were nursing poorly, until I turned 'em "upside down".  One was a baby of a c/section mom -- reason enough right there to put the baby in that pose -- who had very unusual areaolae, which the baby was grasping poorly and macerating.  Down over the shoulder mom and baby felt much more comfortable, and the baby nursed more vigorously and for longer periods of time without fighting with the breast.  

The second case was a vag baby, with no meds on board, but fussed alot at breast, no matter whether he was upright, crosswise, or across mom's body.  Lengthwise along mom's body and side-lying were out of the question for mom, since she had a separated symphesis, and could not bear the babies weight against her pubic area, and she could not roll from side to side.  Much less find a comfortable position on her side.  But the upshot is, the baby loves leaning across her shoulder to reach down for the nipple, and again has had the best, longest feeds since.

We do have to factor in that these kids are now day 3, and there is more milk, but the fact is, both of them seemed not to like any of the more traditional holds, even today.

Anyway, thanks Ozzies, for the descriptions in past posts.  It got so I could visualize it, and the babies went at it like it was the most natural things for them.
