At last I can give some info. instead of only asking!

I received a copy of the following paper recently:
"Back to Nature?  Historical and Cross-cultural Perspectives on
Barriers to Optimal Breastfeeding", C. Obermeyer and S. Castle.
Medical Anthropology, Vol. 17, pp. 39-63.

It contains a section on Breastfeeding Patterns and the Insufficient
Milk Syndrome.
