Hello to all, I'm coming out of lurk-dom to post a question for a client.
I'm a RN,CBE working in an NICU setting and also in a low risk level 1
nursery here in Denver ( a formerly sunny city that today resembled a
much damper local!! ). I've had only phone contact with this mom and got
her permission to ask my fellow Lactnetters for some help with her
concerns. She reports an almost overnight drop in her milk supply and
has worked hard to recoup her supply but we wanted to leave no stone
unturned so here's her story. Her infant is 3 months old and has
exclusively BF since birth with "normal" weight gain. Her baby did begin
sleeping longer intervals at night, 10 hour one night then 13 hours the
next. Mom noted dramatic changes in her production the next day, began
every 2 hour feeds followed by pumping with a double kit and a L**tin*
pump, for less than one ounce (she had in the past been able to pump
after feeds for 7-8 ounces). She saw her MD--normal prolactin levels but
high thyroid reported. He said "graves disease, stop BFing and get ready
to take meds for the rest of your life." :-(. Smart Mom called around
and found an Endocrinologist willing to help her without sacrificing her
BF relationship with her child. His conclusion is thyroiditis, and told
her up to 16% of PP mom's thyroids overproduce in the first several weeks
after delivery, no meds needed and keep on BFing :-) !!
Her concern is that her supply still seems boarder line. She tried
Reglan, but took only 7 doses and stopped due to some unpleasant side
effects. She is taking Fenugreek, but not yet seeing an effect. She
does co sleep the last half of the night, and tries to watch her fluid
intake as well as eating "healthily". She is currently BFing every 3-4
hours for 5-15 min each side and still pumping 3-4 times a day, 10 min.
each time. She feels her supply is OK now but worries about a recurrence
of this drop in volume esp. since she's not sure what caused it. I gave
her high marks for her persistence, told her to keep up the great work
and that I'd ask around for any more info. available. I checked
Lawerence and BAB but found no info on low supply with high thyroid
function. Any ideas??? Have we over looked something??
